Q. Can I fill anything out before I come to my appointment?
A. Yes, you have a couple of options in order to do this.

1) You may print the following forms based on your appointment type and bring them with you to your scheduled appointment.

Visit our Home Page or our Patient Login Page in order to download and print the forms to complete prior to your visit.

2) You can contact us with your email address and we can enable you to login to our Patient Portal in order to communicate with us electronically. Click on the words “Patient Login” on the upper right corner of our website to learn more.


Q. When do I need to schedule my first OB/pregnancy appt?
A. We typically schedule an initial appointment with one of our nurses/CMA’s in order to begin your care. This appointment usually lasts 30 to 45 minutes and consists of a question and answer session and blood draw.

The first appointment with your physician is typically between 8 and 10 weeks gestation and usually includes an ultrasound along with the doctor visit.


Q. Do you accept my insurance?
A. We accept most insurance plans and file claims on your behalf. Please contact your insurance company to see if we are In-Network with your particular plan. Most insurance companies have an online provider directory and/or a member customer service.

Q. Do you accept Medicaid?
A. Yes and No.

We do accept Medicaid primary for GYN services on a referral basis ONLY. Your primary care physician MUST call to refer you to our practice.

**Routine annual exams should be scheduled with your primary care physician.

We do NOT accept Medicaid Family Planning Waiver. Please ask about our Medicaid Policy.

Q. My Insurance paid some of my bill, but I still have a large balance. Can I arrange a payment plan?
A. Yes, we offer monthly payment arrangements not to exceed 6 months via a debit or credit card kept on file with our office. You choose the day of the month for your payment to be run.

Q. Do you offer self pay discounts?
A. Yes, we offer a 20% discount if services are paid for in full on the date services are rendered.

Q. What if I cannot pay in full on the date of service?
A. You would be responsible to pay half of the charges on the date of service and would arrange a payment plan via a debit or credit card to be run monthly for no longer than 6 months. The self pay discount would no longer apply.
